Before Your First Appointment

If you are scheduling a consultation with Dr. Hutchinson, provide your name, phone number, email address, date of birth, and full address.

  • My off-site assistant will call you, or we can arrange the first appointment online.

  • You will get a message from us with a link for setting up your Elation Passport secure messaging account portal and then with some forms to complete online. These first scales  will take some time but will help us put everything together when we meet. 

  • It is also helpful if you are able to send for your records from other doctors or therapists  working with you now or in the past.
